Patient Portal

To provide the safest experience possible for all patients, you are required to complete a patient questionnaire through our secure, easy-to-use DAOPM Patient Portal prior to your first office visit. This information can no longer be completed in our office due to social distancing guidelines and regulations. You will also be able to send and receive messages with our staff through the portal.

Additionally, you will use the portal to:

  • Review and verify your contact information


  • Input past medical history/medications/allergies/social history/preferred pharmacies


  • View finalized visit summaries, lab results and patient handouts


To access the portal, please click the button below or type into your internet browser address bar (do NOT use "www" before the web address). We recommend using the Google Chrome web browser for the best experience.

Enter your username and password.  If you haven’t established a password, please click on RESET PASSWORD. Enter your username, date of birth and last name. You will receive an email, follow the instructions and log in to your portal  Upon accessing the portal, visit the "My Health" tab found near the top left corner of your screen. Please note: There are NINE information tabs accessible on the left side of the screen that must be completed. Remember to SAVE your information prior to progressing to the next tab!  After you have complete Family History, click save and log out of the portal by clicking on your name in the upper right-hand corner and clicking on Log Out.

Once there:

From the log-in screen, input your username and password
Once you've loggin in, you will be prompted to input a personal password
You with then have access to the portal for the information outlined above

Note About Lab Results:

Today's biopsies will be processed and your test results may be available in 10 to 14 days on the DAOPM secure patient portal. You may access your results any time of day via Internet access to the secure DAOPM Portal. Accessing the Portal is described on the front of this Information Sheet.

Once on your Portal site, select "Visit info" just above your name on the top left of your Portal. Once selected you will have access to information regarding your visits to DAOPM and any Lab Biopsy results that have been completed. Highlight the "blue' date/time and review the results.
